Leaving high school can be both exciting and scary for a young adult with a disability. Although it can be a new beginning, it can also be the end to different services and supports. It is important to spend time preparing for the transition to adult life and finding out information about future needs and opportunities.

Transition is a process of changes. These changes may affect healthcare, education, employment, benefits, and supports. This section includes information on transition planning and goal setting.
